Role Overview The Wellness Nurse (LPN/LVN) is responsible for supporting resident health and ensuring clinical quality in a senior living environment. Key duties include conducting wellness assessments, monitoring changes in resident health, supporting medication management, and performing MAR/TAR audits. The role involves communicating effectively with families, physicians, and care teams, as well as documenting care activities to ensure compliance with regulations.
Additional responsibilities include monitoring incidents such as falls, supporting infection control initiatives, and participating in clinical quality improvement efforts. The Wellness Nurse also trains and supports care team members to maintain high standards of care.
Qualifications & Skills
- Active LPN/LVN license
- Over one year of experience in senior living or clinical settings
- Strong assessment, documentation, and communication skills
- Knowledge of medication administration and compliance protocols
- Effective problem-solving abilities
