Wellness Coordinator

About This Role

Working as a Wellness Coordinator involves providing non-clinical care coordination and administrative support to members with behavioral health and substance abuse needs. This position focuses on identifying care gaps, addressing social determinants of health, and developing individualized care plans to improve members' access to treatment and resources.

Responsibilities

• Manage inbound and outbound member communications
• Document interactions in multiple electronic systems
• Coordinate information with clinical team members
• Educate members on behavioral health benefits
• Screen members for care gaps
• Provide referrals to network providers and community resources
• Offer administrative support to the Behavioral Health Case Management team
• Potentially travel to provide direct patient care
• Communicate with healthcare providers as needed
• Perform other duties as assigned

Requirements

• High School Diploma or GED
• Minimum 2 years of managed care or call center experience
• At least 1 year in a medical or behavioral health setting
• Proficiency with computer applications including Adobe and Microsoft Office
• Experience handling sensitive information confidentially
• Access to a quiet, secure workspace at home for telecommuting
• Reside in the Kansas City, MO area with approved high-speed internet

Nice to have:

• Bachelor
• Casework experience in hospitals, social services, or community mental health
• Experience with dual diagnosis and state waiver programs
• Knowledge of coordinating benefits and community resources
• Strong problem-solving and initiative skills
• Valid Missouri driver
