About This Role
This role involves coordinating resident care, clinical services, and overall wellness programming within the community. The Wellness Coordinator supports residents' well-being, partners with the care team, and blends hands-on nursing with coordination, communication, and leadership.
Responsibilities
- Support resident assessments, care planning, and ongoing wellness monitoring
- Coordinate clinical services to ensure resident needs are met in a timely and compassionate manner
- Administer medications and treatments in accordance with physician orders and state regulations
- Monitor changes in resident condition and communicate updates to families and providers
- Ensure clinical documentation is accurate, complete, and compliant with state and company standards
- Assist with audits, surveys, and quality assurance initiatives
- Support infection control practices and medication management protocols
- Help maintain survey readiness at all times
- Collaborate with caregivers, leadership, and interdisciplinary team members
- Provide guidance, education, and support to caregiving staff as needed
- Serve as a clinical resource to promote best practices and resident-centered care
- Foster a positive, team-oriented environment
- Serve as a point of contact for resident wellness questions and concerns
- Communicate effectively with residents, families, providers, and internal teams
- Support admissions, move-ins, and transitions of care as applicable
Requirements
- Long-term care, assisted living, or senior living experience preferred
- Experience with medication administration and resident assessments
- Strong organization, time-management, and follow-through skills
- Compassionate, resident-focused approach to care
- Clear and professional written and verbal communication
- Ability to multitask and adapt in a fast-paced environment
- Proficiency with electronic medical records (PointClickCare experience preferred)
- Ability to lift up to 30 lbs and assist with resident care as needed
- Ability to walk, stand, bend, and lift throughout the shift
- Flexible availability, including weekends, holidays, and on-call as needed
