About the role
We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes, following physician plans and ensuring quality outcomes. Key Responsibilities Include
- Delivering and coordinating patient care according to physician orders.
- Assessing patient conditions, documenting activities, and reporting changes.
- Updating care plans and implementing appropriate nursing actions.
- Recognizing and responding to patient needs to prevent risks.
- Educating patients and families on disease management and self-care.
- Maintaining compliance with regulations, infection control, and confidentiality standards.
Qualifications And Skills
- Active, unencumbered LPN/LVN license in the practice state.
- Current CPR certification and TB test within last 12 months.
- Six months of clinical nursing experience preferred.
- Reliable transportation, attention to detail, good communication, and time management skills.
- Ability to lift 50 pounds, reposition patients, and respond to emergencies.
Physical And Environmental Requirements
- Proficiency in English, able to travel, and work in diverse environments.
- Exposure to blood, bodily fluids, and infectious agents may occur.
- Prolonged walking, standing, bending, and climbing stairs required.
