About the role
Role: Certified Nursing Assistant (CNA) providing home care services.
Responsibilities
- Deliver direct patient care, including personal hygiene, mobility assistance, and basic nursing tasks as directed by healthcare professionals.
- Assist with transfers, repositioning, and ambulation to ensure patient safety.
- Prepare meals considering client preferences and dietary needs.
- Offer companionship and engage clients in activities tailored to their interests.
- Perform light housekeeping duties and maintain accurate documentation of care provided.
- Monitor and report changes in patients' physical or psychosocial conditions.
- Follow the Plan of Care strictly, and use EVV systems to document tasks.
- Uphold patient confidentiality, privacy, and rights, and report any concerns or abuse immediately.
Qualifications And Skills
- Maintain current certification with DFS.
- High school diploma or GED preferred.
- Effective communication skills and ability to follow directions.
- Physical ability to lift approximately 50 lbs, bend, and stand for extended periods.
- Compassionate attitude toward patient care.
Key Duties: Provide in-home care, support patient independence, document care accurately, and collaborate with team members to ensure quality healthcare delivery. Travel within designated areas is required.
