About the role
Empath Home Health, a member of Empath Health, is seeking a compassionate Weekend Baylor RN (Home Health) to work with our Home Health team to provide skilled nursing care to patients in their homes throughout the Ocala area (Marion, Lake, & Sumter counties). The Weekend Baylor RN will work Friday-Sunday (3 days per week) OR Friday-Monday (4 days per week).

Locations: Home health visits in the Ocala area (Marion, Lake, & Sumter counties)
Schedule: This is a Weekend Baylor role requiring full weekends**. You decide which schedule below fits your life!**
- 4 days (Friday-Monday, Thursday-Sunday, OR Saturday-Tuesday)
- 3 days (Friday-Sunday OR Saturday-Monday)
What you'll Do
- Performs initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
- Initiates the plan of care and revise as necessary.
- Provides nursing services and/or treatments indicated on plan of care.
- Counsels and educates the patient and family on disease process, self-care techniques and prevention strategies.
What You'll Need
- Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
- BSN or MSN preferred.
- Minimum one year experience providing direct patient care; previous experience as a home health RN preferred.
- Home Health RN experience required
