Job Description:
The Role: Triage RN
If you have a passion for working with the geriatric population and a desire to work in an office with a small and collaborative team, this may be the perfect position for you.
The nurse who takes this role will:
- Take calls from patients/caregivers requesting assistance. Provide advice, referrals, and patient education. For symptomatic patients, determine the necessity and type of care needed and advise the caller regarding appropriate care.
- Perform a nurse-driven annual wellness visit to include: patient intake, chronic condition management, and cognitive testing.
- Perform administrative duties to coordinate patient care, including data collection, answering phones, scheduling tests/procedures and appointments, calling in prescriptions, determining insurance coverage, and billing and receiving payments.
What You'll Do:
- Elicit questions, concerns, and information from the caller to assess symptoms, determine acuity, identify appropriate algorithm/protocol, and develop a treatment plan.
- Provide professional advice, education, and options based on assessment, established algorithm/protocol, and clinical judgment. When appropriate, dispatch appropriate discipline personnel or emergency response units.
- Provide referrals to healthcare providers and community resources when appropriate, including locations, hours, and contact information. Assist caller with scheduling appointments at referral sources when appropriate.
- Perform administrative duties to coordinate patient care, including data collection, answering phones, scheduling tests/procedures and appointments, calling in prescriptions and determining insurance coverage. Document calls, responses, and referrals. Perform follow-up calls to evaluate effectiveness of care and need for additional care.
Your Qualifications:
- Active Maine RN licensure or ability to obtain same in a timely fashion
- 1 year of RN experience preferred
- Previous practice experience, helpful
- Strong assessment skills, especially over the phone
- Excellent teamwork, communication, and collaboration skills
- Proven and demonstrated critical thinking skills
- Ability to react calmly, objectively, and effectively in emergency situations
