About the role
We are seeking a Skilled Nursing Facility (SNF) Physical Therapist to provide patient-centered rehab services focused on restoring mobility, independence, and quality of life for short-term and long-term care patients.
In this role, you'll evaluate and treat patients recovering from illness, injury, or surgery, develop and update individualized plans of care, and deliver 1:1 skilled therapy primarily to Medicare patients. Collaboration with nursing, occupational, speech therapists, and rehab leadership is essential, along with documenting evaluations, progress, and discharge summaries. Educating patients and families on mobility and safety is also key.
- Qualifications** include a Doctorate or Master’s Degree in Physical Therapy, an active or pending state PT license, and preferably SNF experience, with strong clinical reasoning and communication skills.
