Job Description:
The Physical Therapist (PT) will provide in-home rehabilitation services to patients across various locations, including Assisted Livings, Senior Apartments, and Board and Care homes. The PT will handle 3 to 5 patients a day, including Start of Care visits and evaluations. The role involves patient care, documentation, and coordination of treatments without a point system.
Responsibilities:
- Conduct home health visits and provide physical therapy treatments.
- Supervise up to 2 different PTAs/COTAs.
- Coordinate and plan care as per patient needs within designated territories.
- Work effectively within the Mercy area hospitals and Community doctors service network.
Requirements:
- Current and valid CA PT license.
- Minimum of 2 years experience in a healthcare setting, home health experience preferred.
- Required certifications: BLS & NPI #.
- HomeCare Home Base (HCHB) experience preferred.
- Possession of a current driver's license and proof of automobile insurance.
- Good oral and written communication skills; Good organizational skills.
