Home Health PT Opportunity – Anchorage, AK

Ardor Health Solutions is seeking an experienced Home Health Physical Therapist for a 13-week travel assignment with Providence at Home with Compassus in Anchorage, Alaska .

This is an excellent opportunity for a PT who enjoys autonomy, one-on-one patient care and managing their own Home Health caseload , without weekend or on-call requirements.

Assignment Details

• 13-week contract
• Monday–Friday, 8:00 AM–5:00 PM
• 40 guaranteed hours per week
• No weekends
• No on-call
• Territory: Anchorage municipality
• Productivity: 30 points per 40 hours
• EMR: Homecare Homebase (HCHB) preferred

Experience Requirements

• Minimum 1 year of Home Health experience required
• OASIS experience required
• Homecare Homebase experience preferred
• Active Alaska PT license
• Current BLS

Home Health Responsibilities

• Complete in-home PT evaluations and reassessments
• Develop individualized plans of care
• Provide 1:1 patient care
• Gait, balance and transfer training
• Strengthening and functional mobility
• Fall prevention and home-safety education
• Patient and caregiver education
• Complete required Home Health/OASIS documentation
• Coordinate care with nursing, physicians and other members of the interdisciplinary team
