Telephone Triage Nurse

About This Role

The Telephone Triage Nurse is a key part of an interdisciplinary team, delivering high-quality, compassionate care by assessing and prioritizing patients' medical needs over the phone, following established protocols. This role suits a nurse committed to ethical, inclusive, and patient-centered care within a supportive environment.

Responsibilities

• Triage patient calls to determine the appropriate level of care and coordinate with care teams
• Schedule appointments for new and returning patients across health services, excluding Family Planning and WIC
• Observe, document, and report patient conditions and responses to medications and treatments
• Provide education to patients about their current health conditions and care as appropriate
• Consult with clinical leadership regarding unresolved patient concerns and communicate effectively with providers
• Use the nursing process, maintain accurate patient records, report diagnostic results, and follow nursing policies, procedures, and standing orders
• Support flexible nursing assignments, attend meetings and trainings, participate in committees, and maintain professional knowledge and affiliations
• Protect patient confidentiality, demonstrate compassion and teamwork, respect individual differences, and perform additional duties as needed

Requirements

• Minimum graduate of an accredited school of licensed practical nursing
• Possession of an Indiana State Nurse license
• CPR certification and TB certification, required to be maintained through ODHS post hire
• Ability to react calmly in emergency situations
• Ability to read, understand, and follow detailed written and oral communication
• Ability to learn to use and update electronic health records and other online systems

Nice to have:

• Two years of professional nursing experience in clinic/office setting
• 1-2 years of telephone triage nurse responsibilities

Physical And Environmental Requirements

• Frequent exposure to communicable diseases, toxic substances, medicinal preparations, and other conditions common to clinic environment
• Requires corrected vision and hearing to normal range or use of approved adaptive equipment
• Requires standing, walking or sitting for long periods of time
• Occasional bending, stretching or lifting up to 40 pounds
• Requires the ability to work under stressful situations
