Telehealth Triage Registered Nurse

About This Role

This fully remote nursing position is for an experienced bilingual Registered Nurse who enjoys critical thinking, patient education, and making meaningful clinical decisions. You will collaborate with a supportive team of experienced nurses, advanced practice providers, and physicians who value your expertise. Every day, you'll help patients navigate urgent health concerns, provide evidence-based clinical guidance, and ensure they receive the right level of care at the right time.

Responsibilities

• Perform comprehensive telephone assessments
• Evaluate patient symptoms and determine the appropriate level of care, including: home care, primary care follow-up, same-day appointments, urgent care, emergency department referral, transfer to an Octiva Healthcare provider when appropriate
• Provide evidence-based nursing advice and patient education
• Educate patients regarding medications, symptom management, warning signs, and follow-up recommendations
• Coordinate medication refill requests according to organizational policies
• Collaborate with physicians, nurse practitioners, clinic staff, and internal clinical teams to ensure continuity of care
• Escalate urgent and emergent clinical situations according to established workflows
• Review patient histories within multiple Electronic Medical Record (EMR) systems
• Maintain accurate, timely, and detailed clinical documentation
• Deliver compassionate, patient-centered care while maintaining exceptional customer service
• Perform additional duties as assigned

Requirements

• Graduate of an accredited School of Nursing
• Active California Registered Nurse License
• Active Compact (Multi-State) Registered Nurse License
• Active, unrestricted RN licenses in good standing
• Minimum of three (3) years of recent Registered Nurse clinical experience
• Fluent in both English and Spanish (written and verbal)
• Strong clinical assessment and critical thinking skills
• Excellent telephone communication skills
• Ability to make sound clinical decisions in a fast-paced environment
• Strong understanding of symptom assessment and urgent/emergent care criteria
• Proficiency with multiple Electronic Medical Record (EMR) systems
• Ability to prioritize multiple clinical situations effectively
• Strong attention to detail and documentation accuracy
• Ability to work independently while collaborating with a multidisciplinary team
• Commitment to providing compassionate, patient-centered care
• Dedicated, quiet home office free from distractions
• Comfortable navigating multiple software platforms simultaneously
