Montgomery County, PA Street Medicine program is seeking a Psych. Nurse Practitioner based out of Pottstown Hospital. This grant funded position offers an exceptional opportunity to provide impactful health care as the leader of a passionate team of providers, support staff, and community partners.
The Psych. NP will be a licensed Nurse Practitioner, responsible for delivery of high quality clinical care to the homeless and housing insecure patient population in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). Job Responsibilities
- Develops virtual clinic SOPs.
- Coordinates care with other providers.
- Develops Street Medicine Consult SOPs.
- Reviews and monitors clinical records to ensure compliance with professional standards.
- Assists in programmatic administration and development of services, policies and procedures, compliance needs, Continuing Medical Education Committee
- Completes initial clinical evaluation.
- Develops treatment plan.
- Documents in Epic.
- Completes discharge summary.
- Orders and interprets diagnostic and therapeutic tests, relative to patient’s age-specific needs; Prescribes appropriate pharmacologic and nonpharmacologic treatment modalities; Monitors the effectiveness of interventions; Collaborates with multidisciplinary team members by making appropriate referrals.
Education Requirements
- Master's Degree or BSN and currently enrolled in an MSN program
