Role Overview: The Speech Therapist in Home Health assesses, evaluates, and treats patients with speech, language, swallowing, and cognitive impairments. They develop and implement treatment plans, monitor patient progress, and collaborate with healthcare teams to improve patient communication and functional abilities.
Key Responsibilities
- Provide speech therapy services within legal and professional scope, adhering to the plan of care.
- Assess patient functionality using diagnostic tests and assist in developing therapy plans with physicians.
- Treat patients to enhance effective communication and express thoughts appropriately.
- Observe, document, and report patient responses and condition changes to supervising healthcare providers.
- Educate patients, families, caregivers, and healthcare team members on speech therapy techniques and participation.
Qualifications & Skills
- Licensed in Speech Therapy in the state of residence.
- Current CPR certification.
- Valid driver’s license, reliable transportation or access to public transit.
