About the role
Role: Speech Language Pathologist providing one-on-one care to older adults in private homes and senior living communities, including virtual care options.
Responsibilities
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support patient independence and wellness.
- Document clinical progress accurately within electronic medical records.
- Collaborate with interdisciplinary teams including PTs, OTs, and caregivers.
- Educate patients, caregivers, and staff on safety, function, and aging in place.
- Utilize telehealth and virtual care to extend access and flexibility.
Qualifications & Skills
- Valid state licensure and eligibility or certification (ASHA CCC).
- Degree from an accredited Speech Language Pathology program.
- Proficiency with computers and EMR systems.
- Current CPR certification.
- Ability to meet physical demands including lifting, repositioning patients, and driving.
