Social Worker Service Coordinator - Homeless

About This Role

Working with an interdisciplinary team, the Social Worker Service Coordinator participates in the ongoing collaboration, assessment, planning, implementation, coordinating, monitoring, and evaluation of individuals and families to help them achieve optimal health. This role provides case management services to a targeted population, assisting with coordination and linkage to applicable health and community-based services, and conducts outreach efforts to locate and determine eligibility for individuals experiencing homelessness.

Responsibilities

• Work with the interdisciplinary team across the continuum of care in participating in the ongoing collaboration, assessment, planning, implementation, coordinating, monitoring, and evaluation of the individual/family to gain optimal health
• Provide case management services to a targeted population to assist with coordination and linkage to applicable health and community-based services
• Conduct outreach efforts to locate and determine eligibility for individuals experiencing homelessness
• Promote effective and efficient utilization of health care resources with a focus on health promotion
• Work within the program directives of the funding source, within the philosophical framework set by the Board of Directors, and within the existing network of community service providers and natural helpers

Requirements

• Master's Degree in Social Work from a CSWE (Council of Social Work Education) accredited school
• A current Hawaii State driver's license and daily access to an automobile
• CPR certification and biannual maintenance
• Ability to adhere to OSHA standards and safety precautions in the community

Nice to have:

• LSW (licensed in the State of Hawaii), experience will be considered in lieu of licensure
• A minimum of two (2) years' experience practicing as a Licensed Social Worker
• A minimum of three (3) years' experience practicing as a Case Manager
