Role Overview The Hospice Social Worker assesses the psychosocial needs of patients and families, providing resource support, short-term counseling, and crisis intervention in collaboration with interdisciplinary teams. Responsibilities include conducting assessments, developing bereavement care plans, interpreting cultural and spiritual beliefs, and documenting patient progress. They facilitate patient decision-making, support end-of-life care, and assist with advanced directives, transportation, and resource referrals such as financial aid and elder abuse reports.
Qualifications Candidates must hold a Master’s Degree in Social Work, be licensed as an LSW or LCSW, and possess a valid driver’s license with auto insurance. A minimum of one year healthcare social work experience is required.
