About the role
Role Overview: The Home Services Social Worker assesses psychosocial needs of patients and families, providing interventions such as resource support, short-term counseling, and crisis management in collaboration with interdisciplinary teams.
- Conducts psychosocial assessments to identify emotional, social, and environmental strengths and challenges related to diagnosis and treatment.
- Develops bereavement care plans and interprets cultural and faith traditions impacting care.
- Educates patients and families, overcoming barriers, and facilitating understanding of advanced directives and life care decisions.
- Documents assessments, interventions, and care plans in electronic medical records, ensuring timely updates and compliance.
- Implements and evaluates social work plans to enhance family support, resource utilization, environment, dignity, and coping skills.
- Addresses indicators of imminent death and supports patient/family needs during end-of-life scenarios.
- Assists with referrals for transportation, food, housing, financial aid, Medicaid, elder abuse, and other community resources.
- Practices with sensitivity to palliative care principles, ensuring support aligns with hospice benefits and patient preferences.
Qualifications & Skills: Master’s Degree in Social Work, licensure as LCSW or LSW, experience in hospice care, and valid driver’s license and auto insurance. Strong assessment, communication, and documentation skills, with a focus on patient-centered, culturally sensitive care.
