Social Worker

Villa Francis Home for the Aged

Key Responsibilities

Conduct psychosocial assessments of residents and identify care needs.
Provide counselling, emotional support and case management services to residents and their families.
Facilitate admission, discharge planning and care transitions.
Assist residents and families in accessing community resources, financial assistance schemes and support services.
Collaborate with multidisciplinary teams including doctors, nurses, therapists and care staff to develop and implement care plans.
Mediate and resolve family-related concerns and care issues.
Maintain accurate case records and documentation in accordance with organisational and regulatory requirements.

Requirements

Degree in Social Work.
Valid registration with the Singapore Social Workers Association (SASW).
Minimum of 5 years of relevant experience in social work, particularly in geriatrics, elderly care or nursing home settings.
Strong interpersonal, communication and counselling skills.
Ability to work independently and collaboratively in a multidisciplinary environment.
Proficient in case management and report writing.
