Social Service Coordinator

About This Role

Supporting patients and their families in coping with the physical, social, emotional, and economic challenges of illness and hospitalization is the core of this role. The Social Service Coordinator works under the direction of the Manager of Care Management to assess patient needs, develop interventions, and create comprehensive discharge plans. Post-discharge follow-up is also an integral part of this position.

Responsibilities

• Work with patients, families, staff, and outside agencies to address the psychosocial needs of patients and families
• Assess patient needs
• Develop and implement appropriate interventions and a comprehensive discharge plan
• Assist patients and families in coping with physical, social, emotional, and economic issues related to their illness, treatment, hospitalization, and/or discharge
• Conduct post-discharge follow-up of the care provided
• Demonstrate service excellence and patient and family centered care by showing respect, honesty, fairness, and a positive attitude toward all customers
• Maintain confidentiality
• Demonstrate dependability, including attendance and punctuality
• Be accountable, take initiative, and ownership of issues
• Display a professional demeanor and represent the hospital in a positive way
