As a Senior Care Coordinator (Home Care), you will be responsible for the following duties:
- Collaborate closely with the multidisciplinary care team to deliver holistic and patient-centred care.
- Empower patients and caregivers to actively manage their health through digital and technology-enabled healthcare tools.
- Promote awareness of and connect patients to appropriate community and healthcare resources to improve health outcomes.
- Build and maintain positive relationships with internal stakeholders, patients, and caregivers.
- Support preventive healthcare initiatives by encouraging participation in health screenings and vaccinations.
- Gather and document relevant social determinants of health to facilitate appropriate care planning and interventions.
- Coordinate patient care activities using IT systems to track, document, and monitor relevant clinical indicators.
- Perform basic health assessments and measurements while supporting preventive care initiatives.
- Provide basic health education on healthy lifestyle practices, physical and social activities, and self-care.
- Guide patients on the appropriate use of home monitoring devices such as blood pressure monitors, glucometers, and weighing scales.
- Educate patients on recommended vaccinations and relevant national healthcare programmes.
- Carry out other ad hoc duties as assigned.
Requirements:
- Min. Diploma in Health Science, Health Management or equivalent field.
