Job Summary: Deer Meadows Home Health Services is seeking a dedicated and compassionate RN Registered Nurse Case Manager to join our team. RN Registered Nurse Case Manager Responsibilities:
- Assess patients' physical and emotional needs in their homes and develop individualized care plans to ensure optimal health outcomes
- Coordinate and collaborate with patients, families, caregivers, and healthcare providers to ensure seamless continuity of care
- Provide skilled nursing care, including wound care, medication management, and education to patients and caregivers
- Conduct regular home visits to monitor patients' progress, identify potential breakthroughs, and make adjustments to care plans as needed
- Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate transmission of patient information
- Develop and maintain accurate and up-to-date patient records and reports
- Participate in quality improvement initiatives to improve patient outcomes and satisfaction
- Collaborate with interdisciplinary teams to develop and implement patient-centered care plans
RN Registered Nurse Case Manager Requirements:
- Current licensure as a Registered Nurse in the State of Pennsylvania
- Prior experience in home health care, case management, or a related field
- Strong clinical and case management skills, with the ability to assess patient needs and develop effective care plans
- Excellent communication, interpersonal, and organizational skills
- Ability to work independently and as part of a team
- Possess a valid driver's license and reliable transportation
