What You Will Do
- Practice independently in the community as part of an interdisciplinary care team.
- Deliver personalized nursing and care management to patients in their home or care facilities.
- Constantly evaluate evolving patient needs and respond with plan of care adjustments.
Qualifications Licenses and Certifications: A license and current registration to practice as a Registered Professional Nurse in New York State required and
Education Bachelor's Degree in Nursing OR Associate’s degree in Nursing required
Work Experience Four years’ clinical home care experience required and
Minimum of two years full-time clinical home care experience required and
Minimum of one year of experience as a field staff nurse in VNS Health Home Care, which includes ability to manage/document cases with a minimum of supervision, identify/assess/formulate/implement action plan to address problem and evaluate outcome preferred and
Pay Range USD $126,413.00 - USD $160,188.00 /Yr.
