About the role
Role Overview: The case management nurse collaborates with healthcare teams to develop, implement, and report on patient care plans, ensuring optimal outcomes. They act as a liaison among patients, families, clinicians, and other stakeholders, providing expert consultation on patient-specific issues.
Key Responsibilities
- Coordinate patient care across disciplines, monitoring appropriateness and timeliness.
- Ensure care plans align with patient needs, reviewing diagnostics, treatments, and procedures.
- Discuss resource utilization, treatment plans, and discharge strategies with physicians, focusing on complex and high-utilizer patients.
- Manage patient transfers to tertiary centers, ensuring continuity of care.
- Collect data for regulatory compliance and oversee frequent ED visitors through focused reviews.
- Perform additional duties as assigned.
Qualifications & Skills
- Registered Nurse (RN) licensure required.
- Bachelor’s Degree in Nursing preferred.
- Experience in care management and case management certification preferred.
- Strong communication, coordination, and analytical skills to manage complex patient cases.
