About the role
Role Overview Provides exceptional patient care by supporting the development, implementation, and reporting of the case management program. Acts as a liaison among patients, families, healthcare providers, and the care team to optimize outcomes and ensure coordinated, appropriate care.
Responsibilities
- Leads patient care coordination within the multidisciplinary team, monitoring care appropriateness and timeliness.
- Ensures care plans align with patients' clinical needs, reviewing diagnostics, treatments, and procedures.
- Collaborates with physicians on resource utilization, treatment plans, and discharge planning, focusing on complex and high-utilizer patients.
- Manages patient transfers to tertiary centers, ensuring seamless information exchange for ongoing care.
- Collects data for regulatory compliance and reviews frequent ED utilizers for quality improvement.
- Performs additional related duties as assigned.
Qualifications & Skills
- Registered Nurse (RN) licensure in the practice state: Required
- Bachelor's Degree in Nursing (BSN): Preferred
- Experience in care management: Preferred
- Case management certification: Preferred
