📌 RN Home Health - Brunswick, GA (Local or Traveler) Assignment Details
- Contract Length: 13 Weeks
- Schedule: Monday - Friday, 8:00 AM - 5:00 PM (40 hours/week)
- Travel Area: Patient Service Area: Glynn, Camden, Charlton, Brantley, Wayne, Long, and Mcintosh Counties
- Productivity Units: 30 units
Responsibilities
- Deliver skilled nursing care in patients' homes, following established care plans.
- Complete comprehensive assessments, including OASIS documentation.
- Administer medications, perform wound care, and provide patient/family education.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Ensure timely and accurate documentation using Homecare Homebase (HCHB).
- Manage admissions, recertifications, and discharges.
- Participate in on-call rotation as required.
Qualifications

Required:

Active RN license in Georgia (or compact state).

1-2 years' experience as an RN in a clinical care setting.

2+ years of home health experience.

Proficiency with HCHB software and OASIS documentation.

Current CPR certification.

Reliable transportation, valid driver's license, and insurance.
