About the role
Role: The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care, evaluating patient needs, and developing individualized care plans.
Responsibilities: Conduct initial eligibility assessments within 48 hours of referral, reassess patient needs at least every 15 days, initiate and coordinate the plan of care, document patient progress, collaborate with patients, families, physicians, and interdisciplinary team members, and provide care in diverse home settings regardless of demographic factors.
Qualifications: Current unrestricted RN license in the practice state, valid driver's license, insurance, and access to reliable transportation. Preferred qualifications include CPR certification and at least one year of clinical experience.
