About the role
Job Summary The role involves providing day-to-day patient care in the home setting, including
case management, post-hospital assessments, discharges, patient teaching, dressing changes, IV therapy, and general skilled nursing
services according to policies and procedures.
It also includes assisting with safety needs such as lifeline, meals on wheels, and public program instruction, ensuring
timely and accurate documentation through the Epic system.
This per diem position offers as-needed visits, with responsibilities performed in alignment with organizational values and standards.
Qualifications & Skills
- Graduation from an accredited nursing school
- Current state licensure and valid driver's license
- CPR certification
- Preferred: at least one year of related experience
