Job Summary:
As an RN Case Manager, you will be responsible for coordinating and managing the care of patients in various healthcare settings. You will work closely with patients, families, and healthcare providers to ensure the best possible outcomes for patients.
Responsibilities and Duties:
- Perform comprehensive assessments of patients' health and psychosocial needs.
- Develop and implement individualized care plans for patients.
- Coordinate care with other healthcare providers, including physicians, therapists, and social workers.
- Monitor patients' progress and adjust care plans as needed.
- Educate patients and families about their conditions and treatment options.
- Advocate for patients' needs and rights.
- Ensure compliance with all regulatory and accreditation standards.
- Collaborate with insurance companies to obtain authorization for services.
- Document all patient interactions and care plans accurately and in a timely manner.
Qualifications and Skills:
- Registered Nurse (RN) license in the state of practice.
- Bachelor's degree in Nursing (BSN) preferred.
- At least 2 years of clinical nursing experience.
- Certification in Case Management (CCM) preferred.
- Strong communication and interpersonal skills.
- Excellent critical thinking and problem-solving abilities.
- Ability to work independently and as part of a team.
- Proficiency in electronic health records (EHR) systems.
- Knowledge of healthcare regulations and reimbursement processes.
