Role: RN Admissions Nurse
Responsibilities
- Coordinate evening admissions to a skilled nursing and rehab facility.
- Complete comprehensive assessments, review medical histories, verify orders, and develop initial care plans for new residents.
- Collaborate with physicians, therapists, and interdisciplinary team members to ensure safe and timely resident transitions.
- Maintain accurate documentation and support regulatory compliance.
- Educate residents and families on care plans.
- Promote a compassionate, resident-focused environment in a dynamic, clinically driven setting.
Qualifications & Skills
- Registered Nurse (RN) licensure.
- Strong assessment, communication, and collaboration skills.
