Social Care Navigator – REMOTE
Position Overview
We are seeking a compassionate and resourceful Social Care Navigator to support participants in overcoming social and systemic barriers that impact their health and well-being. This is a fully remote position , focused on participant advocacy, Social Determinants of Health (SDOH) navigation, resource coordination, and ongoing support.
The ideal candidate is an empathetic communicator who is comfortable working with vulnerable populations, navigating complex community resources, and building trusting relationships with participants in a virtual environment.
Key Responsibilities
- Manage a Participant Caseload: Take ownership of an assigned caseload, maintain consistent outreach, and ensure participants receive timely and ongoing support.
- Conduct Comprehensive Assessments: Complete structured intake conversations by phone, text, or video to identify SDOH needs such as food insecurity, housing instability, financial challenges, transportation barriers, and healthcare access.
- Connect Participants to Resources: Research and identify local community-based organizations, social service programs, and government benefits that align with each participant's needs.
- Document Participant Activity: Maintain accurate, timely documentation in care management systems, CRM platforms, or EHRs, including outreach efforts, participant goals, referrals, and outcomes.
- Support Longitudinal Care Plans: Follow participants throughout their care journey, conducting regular follow-ups, reassessing needs, troubleshooting barriers, and supporting progress toward established goals.
- Coordinate Warm Handoffs: Collaborate with internal support teams, clinical staff, and community partners to facilitate referrals and ensure continuity of support.
- Advocate for Participants: Serve as a trusted advocate for individuals navigating complex healthcare and social service systems.
- De-escalate Challenging Situations: Use trauma-informed communication and empathy to help participants navigate frustration, barriers, and difficult circumstances.
Required Qualifications
- 2+ years of experience in Care Navigation, Social Work, Health Coaching, Behavioral Health, Community Support, or a closely related field.
- Experience working with low-income, vulnerable, underserved, or chronically ill populations .
- Demonstrated experience using Motivational Interviewing or similar behavioral-change techniques.
- Strong communication, interpersonal, problem-solving, and organizational skills.
- Ability to build rapport and establish trust with participants remotely.
- Comfortable working independently in a 100% remote environment .
- Strong digital literacy and ability to quickly learn and navigate CRM platforms, EHRs, care management systems, and other technology tools .
- Spanish fluency preferred to support a diverse participant population.
