Registered Nurse (RN) - Intermediate Care

Seeking a Registered Nurse (RN) for an Intermediate Care role in a community-based hospital setting. Responsibilities Include

• Providing direct patient care with assessment, diagnosis, planning, implementation, and evaluation.
• Promoting patient and family-centered care, education on illness management, and post-treatment needs.
• Developing and documenting individualized care plans, ensuring clear communication across care teams.
• Assessing physical, psychosocial, and emotional needs, respecting patient preferences and cultural factors.
• Participating in activities that uphold patient safety, quality standards, and regulatory compliance.

Qualifications

• Nursing diploma/associate's degree required; bachelor's degree preferred.
• Active RN license required; no prior experience necessary, though less than two years of experience is preferred.
