About the role
We are seeking a Registered Nurse (RN) Home Health Case Manager, Hospice to provide compassionate, patient-centered nursing care to clients with terminal diagnoses. Key Responsibilities Include
- Developing and implementing individualized care plans in collaboration with an interdisciplinary team.
- Providing education and guidance to patients and families regarding care processes and healthcare decisions.
- Conducting initial and ongoing assessments, evaluations, and informational visits.
- Consulting with physicians and hospice medical leaders to adjust care plans as needed.
- Coordinating care efforts to ensure patient needs are met effectively.
Qualifications And Skills
- Current Missouri or Compact RN license.
- Valid driver’s license with auto liability insurance.
- Strong organizational, managerial, and interpersonal skills.
- Ability to provide effective guidance and support in demanding situations.
