- Contract Length: 14 weeks
- Guaranteed Hours: 40 **Requirements:**
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years of Home health experience
- Proof of current CPR certification
- Current driver’s license
- Reliable transportation and insurance **Roles & Responsibilities:**
- Administer skilled nursing care for clients of all ages in their residence.
- Coordinate care with the interdisciplinary team, patients, families, and referring agencies.
- Assess patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
- Implement, document, and evaluate the Plan of Care (POC) ensuring quality and continuity of care.
- Educate patients and families regarding care, medications, and home safety with tailored teaching methods.
- Supervise and provide clinical direction to home health aides to ensure quality service delivery.
