Seeking a Registered Nurse (RN) Case Manager for a dynamic role in emergency care.
Key responsibilities include: coordinating interdisciplinary care, facilitating patient throughput, ensuring appropriate utilization of resources, and collaborating with patients, care teams, community agencies, and physicians to manage cases effectively. The RN assists in identifying patients suitable for inpatient or observation status in partnership with the Command Center and Utilization Review Coordinator.
Qualifications: Bachelor's Degree in Nursing required; Master's preferred. Minimum of 1 year acute care hospital experience; 2 years in Case Management or Emergency Department experience preferred. Substitutions include 4 years of acute care in lieu of degree and experience. Valid California RN license, BLS certification are mandatory; ACM and CCM certifications are preferred.
Skills & Requirements: strong care coordination, resource management, collaborative teamwork, and compliance with licensing standards.
