Registered Nurse - Primary Care

Role Overview: This is a specialized nursing position within Primary Care, focused on supporting patients with chronic conditions transitioning from hospitalization back to primary care. The RN will conduct post-hospital follow-up calls, in-home visits, and collaborate with primary care providers to develop and monitor individualized care plans. They will manage a panel of patients, providing ongoing follow-up, care coordination, and patient education to promote engagement and self-management of chronic conditions.

Key Responsibilities

• Perform follow-up calls for at-risk patients post-hospitalization.
• Conduct home visits for care coordination, medication reconciliation, symptom assessment, and safety evaluations.
• Partner with primary care providers to implement patient-centered care plans.
• Manage a defined patient panel, ensuring continuous care and progress monitoring.
• Educate and support patients regarding their health conditions, treatment, and medications.

Qualifications & Skills

• Graduate of an accredited baccalaureate nursing program; RN license required.
• Experience in caring for patients with chronic conditions or primary care preferred.
• Strong communication, critical thinking, and team collaboration skills.
• Experience with electronic medical records and computer proficiency.
• Ability to work flexible hours and adapt to unpredictable situations.
• Current state licensure and BLS certification as applicable.
