Role Overview: This position involves coordinating interdisciplinary rehabilitation care, monitoring patient health changes, executing nursing interventions, and supporting discharge planning to promote patient recovery and safety.
Key Responsibilities
- Collaborate with PT, OT, and SLP teams to develop unified treatment plans.
- Assess and communicate critical health changes in real-time to physicians.
- Implement physician orders and direct nursing care to aid physical recovery.
- Assist in discharge planning, providing education and support for safe patient transitions.
Qualifications & Skills
- Current RN licensure from the Tennessee Board of Nursing or multi-state license recognition.
- BLS Provider certification within 1 month of hire.
- Diploma from an accredited nursing program or equivalent professional licensure.
