About the role
Join as a Registered Nurse Case Manager in hospice care, responsible for developing and implementing individualized care plans for patients facing end-of-life. You will conduct initial evaluations within 48 hours of referral, assess patient and family needs regularly, and coordinate care with the interdisciplinary team. Your role includes documenting patient progress, educating families on self-care techniques, and ensuring comprehensive, compassionate support across diverse home settings. Requirements
- Current and unrestricted RN licensure in the practice state
- Valid driver’s license, vehicle insurance, and access to reliable transportation
Preferred Qualifications
- At least 1 year of clinical experience
- CPR certification or ability to obtain within 90 days of hire
