About This Role
Making a difference every day as a registered nurse case manager in home health is the core of this job. It suits a nurse who wants to help patients heal where they feel most comfortable, at home.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care
- Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so
- Promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques
- Supervise LPNs and HHAs
- Complete documentation timely and accurately
- Regularly communicate patient progress to the clinical manager and care team
- Plan and provide staff education
- Perform on-call responsibilities and on-call services to patients/families as assigned
- Participate in clinical development and continuing education programs
- Perform other duties as assigned
Requirements
- One or more years of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.
- Current RN license, specific to the state(s) you are assigned to work
- Current CPR certification
- Valid driver's license, reliable transportation and liability insurance
- If less than 6 months clinical experience as an RN, candidate must participate in RN Intern program
