Care Manager (RN)
Location: Ohio, with Home visits
Work Arrangement: Remote within OH Must reside in Trumbull, Stark, Seneca, Portage, Mahoning counties
Schedule: Monday–Friday | 8:00 AM–5:00 PM EST (training same hours for first 4 weeks)
Position Summary
We are seeking experienced Registered Nurses to support members with complex physical healthcare needs through personalized, high-quality care management. This role focuses on developing individualized care plans, educating members and families, and coordinating services to help members remain as independent and healthy as possible.
Key Responsibilities
- Start each day by reviewing emails, tasks, and voicemails by 8 AM
- Prepare for meetings and scheduled field visits
- Complete 5–7 in-home or community visits per week
- Document all encounters within 24 hours
- Meet required turnaround times for workflows and processes
- Complete assigned training programs on schedule
- Maintain strong communication with interdisciplinary teams
Required Qualifications
Education
- Degree from an accredited school of nursing or Bachelor’s in Nursing
- 2–4 years of related nursing experience
Licensure
- Active Ohio RN license (non-compact state)
Experience
- 2–4 years in nursing, case management, or home health
