Role: Diabetes Nurse Educator
Responsibilities: Provide comprehensive diabetes care, education, and self-management support to patients and families. Ensure adherence to standards of care and accurate documentation. Develop individualized teaching plans for topics such as diet, medication, and self-care. Facilitate education on insulin pump use and implement evidence-based clinical guidelines. Act as a preceptor, support staff development, and lead quality improvement initiatives. Complete clinical assessments and assist with community and organizational projects. Collaborate with healthcare team members to optimize patient outcomes.
Qualifications & Skills: Valid RN license (Illinois), certification as a Diabetes Care and Education Specialist (within 3 years), BLS instructor certification (within 30 days). Required education includes a nursing diploma. Strong clinical assessment, teaching, and leadership skills are essential. Ability to work collaboratively and support patient self-management education is vital.
Key Duties: Deliver patient-centered diabetes education, ensure documentation compliance, support staff development, and participate in quality improvement efforts. Engage in community outreach and organizational projects to enhance care delivery.
