You will work closely with individuals, their families, and whānau to understand their holistic needs and deliver comprehensive, wraparound in-home health services that support independence and overall wellbeing.
This is a highly autonomous role, ideal for a confident and experienced Registered Nurse who enjoys working independently, making clinical decisions, and building meaningful relationships with service users in the community. The role offers a diverse caseload and the opportunity to develop broad clinical knowledge across a range of community health needs.
About Our Role:
- Provide community-based nursing support to long-term service users in their homes
- Conduct phone-based needs assessments and care reviews
- Monitor, review, and support service user goals and progress
- Ensure incidents and accidents are reported accurately and promptly
- Deliver care and support across a range of clinical areas, including:
- Medication management
- Enteral feeding
- Falls prevention
- Pressure injury prevention
- Wound care
- Work autonomously while managing a diverse caseload
- Understanding of ACC and Ministry of Health community contracts is advantageous
Skills & Experience | He pūkenga tou
- Minimum of five years' post-registration nursing experience in New Zealand
- Community nursing experience is desirable
- Current New Zealand Registered Nurse Annual Practising Certificate (APC)
- Experience working with ACC is preferred
- Full, clean New Zealand driver's licence
- Strong communication, assessment, and clinical decision-making skil
