Responsibilities:
- Delivery of all aspects within their scope of practice, RN CM and On Call (7 on & 7 Off), where needed, in all environments that a patient calls home (skilled nursing facility, home, etc)
- Deliver compassionate and comprehensive nursing care to patients facing end-of-life challenges.
- Provides skilled nursing care including administration of medications and treatments as prescribed in the physician’s plan of care for patients in their home setting.
- Conduct thorough assessments to identify patient needs and develop individualized care plans.
- Manage medications and treatments as prescribed by the physician.
- Provide emotional support and guidance to patients and their families.
- Educate patients and families about hospice care and end-of-life issues.
- Collaborate effectively with interdisciplinary team members to ensure coordinated care.
- Maintain accurate and up-to-date patient records.
- Ability to travel up to 100% of the time.
- On call rotation to include 7&7 in addition to their personal case management duties to support the site.

Qualifications:
- MUST have hospice experience in facilities and the community at minimum 2 years. Current and active RN license in the state of employment.
- Bachelor’s degree in nursing (BSN) preferred.
- In-depth understanding of end-of-life care principles and practices and certified home health.
- Exceptional communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to patient care.
- Basic knowledge of iOS operating system.
