We are seeking a Hybrid Quality Care Coordinator / Physical Therapist Assistant (PTA). In this role, you will split your time between clinical, hands-on patient care and administrative care coordination. You will be responsible for creating extraordinary client experiences, managing "best in home care" care plans, and utilizing your physical therapy background to optimize client functional mobility and safety at home.

Key Responsibilities

  • Care Coordination: Develop, maintain, and update comprehensive client care plans; communicate effectively with clients, families, care partners, and medical referral sources.
  • Clinical Oversight (PTA): Utilize your physical therapy assistant background to assess client home safety, contribute to functional mobility goals, and ensure care plans align with physical therapy recommendations.
  • Quality Assurance: Conduct routine and unscheduled in-home supervisory visits to observe caregivers and ensure safe, high-quality, and meaningful care is delivered.
  • Scheduling & Continuity: Manage and schedule client care needs to ensure strict continuity and consistency of care.
  • Office & Team Support: Maintain a calm, positive demeanor in a fast-paced office environment, multitask efficiently, and assist with shared on-call responsibilities outside of normal office hours.

Requirements & Qualifications

  • Education: High school graduate or equivalent (Associate Degree in Physical Therapist Assistant program required).
  • Skills: Ability to work independently, demonstrate sound clinical and business judgment, and multitask effectively.
  • Logistics: Valid driver’s license, a clean driving record, and reliable use of an insured automobile.
  • Availability: Flexibility to assist with on-call duty rotation as required.
