About the role
Role: Pulmonary Disease Navigator, a clinical technologist supporting patients with respiratory conditions.
Responsibilities: Evaluate patients with pulmonary diagnoses such as COPD and asthma, develop and implement education and care plans, including smoking cessation, medication use, oxygen therapy, and pulmonary rehabilitation. Assist with discharge planning, provide patient education tailored to individual needs, perform follow-up calls, and track clinical data to improve outcomes. Collaborate with a multidisciplinary team to ensure seamless care transition and support patient self-management. Participate in quality improvement initiatives.
Qualifications: Associate’s degree in Respiratory Care, Massachusetts licensure, RRT certification, BLS certification, and pulmonary disease educator certification (or obtain within six months). At least three years of experience as a Respiratory Therapist; preferred qualifications include a bachelor’s degree and five years of experience.
Skills & Abilities: Knowledge of cardiopulmonary physiology and respiratory science, effective communication, patient assessment, treatment planning, and ability to perform under stress.
Physical & Other Skills: Ability to walk, stand, transport equipment, and operate respiratory devices; manual dexterity; visual acuity; and strong interpersonal skills.
