Public Health Cardiovascular Evaluator

Job Summary

We are seeking a dedicated and energetic Public Health Cardiovascular Evaluator to join our team to provide evaluation services to two grants within the Indiana Department of Health, Division of Chronic Disease, Primary Care and Rural Health’s Cardiovascular Health Program. Time will be split between both of two grants.

Works under the guidance of the Cardiovascular Health and Diabetes Section Director of the Indiana Department of Health (IDOH) to coordinate and carry out the evaluation activities for Cardiovascular grant and for the WISEWOMAN grant. The Cardiovascular Evaluator will provide evaluation services for both programs to meet grant outcomes and deliverables.

The position is a full-time contract position. This position is a hybrid position, working at least three days per week at the physical location of the Indiana Department of Health, Indianapolis.

Key Functions and Responsibilities:

• Provide evaluation services and write an evaluation plan for each program to assist with meeting deliverables of the Centers for Disease Control and Prevention (CDC) cardiovascular cooperative agreement and Indiana WISEWOMANcooperative agreement.
• Assist the CVHD staff in tracking and measuring performance outcomes for the cardiovascular cooperative agreement and the Indiana WISEWOMAN cooperative agreement.
• Collaborate with the Indiana WISEWOMAN Program Director, the CVHD Section Director and leaders from all involved IDOH sections, to develop the evaluation and performance measurement plan and ensure consistency of implementation across the programs.
• Coordinate evaluation efforts across CVHD staff, internal and external partners, and community contracts whose roles include activities for the Cooperative Agreement.
• Works with CVHD staff to implement evaluation recommendations for program improvement.
• Works with IDOH CVHD Evaluators and Epidemiologists to identify data sources, subpopulations, and communities in Indiana that bear a disproportionate burden of heart disease, stroke, diabetes, and obesity in order to evaluate strategies and interventions and to measure performance.
• Collaborating with Program Director to analyze program data semiannually to inform program partners as they work to strengthen service delivery and improve client outcomes.
• Quantitative and qualitative data collection, analysis, interpretation, reporting and communication as described in the annual evaluation plan to answer annual evaluation questions.
• Providing an update on the evaluation portion of the work plan and annual progress report.
• In conjunction with Indiana WISEWOMAN Program staff, coordinate with the data contractor to integrate changes into the data system to ensure that all necessary information is being collected. This includes both minimum data elements and data that are used more generally for performance management and evaluation purposes.
• Supporting the development of a data management plan, if needed.
• Participating in monthly CDC calls/updates and attending meetings as required by the CDC cardiovascular cooperative agreement and the WISEWOMAN cooperative agreement.
• From these evaluation activities, the evaluator is responsible for completing the following for each program: End of year evaluation report; Evaluation plan for the following year; An updated five-year evaluation and performance measurement plan each year; and Evaluation update Information for the annual report.
• Complete work in compliance with state and federal guidelines and recommendations, budget limitations, and agency policies.
• Performs other duties as assigned.

Minimum Qualifications:

• Bachelor’s Degree in any of the following: public health or community health, health education, or similar degree. Master’s Degree in public health preferred.
• Two years relevant experience needed – in public or community health settings preferred.
• Extensive knowledge in evaluation, logic models, and objective writing.
• Knowledge of public health/health promotion evaluation methods.
• Extensive writing skills for the creation of reports.
• Experience with project coordination in an interdisciplinary/team environment.
• Strong written and verbal communication skills.
• Computer knowledge in Microsoft Word, PowerPoint, and Excel.

Other Relevant Information:

• Requires 5% in-state local travel. Travels in-state as required by IDOH and grant requirements. Out-of-state travel as approved by the Section Director.
• Requires valid Indiana Driver’s License
• Some work outside of regular work hours may be required, including some overnight stays.
• This is a hybrid position at the IDOH office in downtown Indianapolis 3 days per week, with the option of working remotely 2 days per week
• This is a full-time contract position with a starting salary of $60,000/year, commensurate with experience.
