Psychiatric Nurse Practitioner

About This Role

This role involves assessing and treating adults with behavioral health disorders, prescribing and managing psychiatric medications, and delivering patient care in a skilled nursing facility. The practitioner will work closely with primary care teams, specialists, and facility staff to coordinate care and ensure compliance with guidelines.

Responsibilities

• Assess and treat adults experiencing symptoms of a behavioral health disorder according to DMS 5
• Prescribe, recommend, or discontinue drugs and treatment for mental, emotional, or behavioral disorders in accordance with the Collaborative Practice Agreement and the state's Nurse Practice Act
• Deliver patient care based on obligations set forth by the State of South Carolina permissible under their license
• Act as the first-line Psych provider at the SNF facility and represent PSG at the facility
• Evaluate and treat new admissions, follow-ups, and long-term care patients based on PSG guidelines
• Counsel patients using advanced psychotherapeutic techniques
• Liaise with the primary care team and specialists to help coordinate the continuum of care
• Update family members on patient status and medication changes
• Work directly with the facility to maintain compliance with appropriate paperwork for Anti-psychotic medication adherence and initiating Gradual Dose reductions
• Work directly with the facility supervising MD and team to deliver quality care
• Represent PSG in a professional business manner
• Solve acute Psychiatric problems for PSG patients and Management at their facilities
• Responsible for daily documentation within the EMR
• Responsible for daily E/M billing based on guidelines/expectations/opportunities set forth by PSG guidelines for coding
• Work in a professional, approachable, and cordial manner
• Create a partnership between care coordination/oversight and care delivery at the facility
• Participate in improvement initiatives set forth by PSG
• Work directly with MA to help organize and deliver quality care set forth by PSG guidelines
• Maintain appropriate census set forth by PSG guidelines for productivity and Quality outcomes

Requirements

• Graduate of an approved nursing program
• ANCC or AANP with Psych experience or PMHNP
• SC license required
• Registered Nurse certification
• Perform duties within a schedule from 8 AM to 4 PM, Monday to Friday
• CME should be up to date
• Passion to deliver quality care and to help make a difference in the post-acute setting
• Strong clinical background and experience in Psych and well versed in advanced medical knowledge of common chronic disease states including psychosis, delirium and anxiety
• Strong interpersonal skills, team player, and the ability to work independently
• Able to recognize and solve acute problems and treat them appropriately
• Previous experience with EMR and basic computer skills required

• Schedule: 8 AM to 4 PM, Monday to Friday
