About the role
Role Overview As a Registered Nurse Case Manager , you will be responsible for delivering comprehensive hospice care by developing and implementing personalized Plans of Care in collaboration with multidisciplinary teams. Your duties include conducting initial assessments within 48 hours of referral, ongoing re-evaluations every 15 days, and documenting patient progress. You will coordinate care, educate patients and families on self-care techniques, and ensure quality standards are met.
Key Responsibilities
- Perform initial eligibility evaluations and ongoing assessments
- Develop, coordinate, and document the patient's Plan of Care
- Collaborate with patients, families, physicians, and interdisciplinary team members
- Provide education and supervision to patients and families on self-care
- Maintain accurate clinical documentation of patient conditions and responses
Qualifications & Skills
- Unrestricted RN license in the practice state
- At least 1 year of clinical experience
- Current CPR certification
- Valid driver's license, vehicle insurance, and reliable transportation or access to public transit
- Ability to work effectively in diverse home environments regardless of background
