About the role
Role: The Primary Care Nurse collaborates with providers and the clinical team to deliver safe, effective, patient-centered care in a systematic manner, aiming to improve quality outcomes for an assigned patient panel.
Responsibilities: Partner with patients, providers, and team members to ensure equitable and efficient care, working in alignment with organizational standards and values.
Qualifications: Requires an active RN licensure and a diploma in Associate of Applied Science in Nursing. A Bachelor of Science in Nursing is preferred.
Skills & Duties: Provide direct patient care, support clinical workflows, and contribute to quality improvement efforts. Work in a hybrid/remote setting with initial onsite training and onsite work two days per week.
