About the role
Role Overview: The Primary Care Patient Care LPN supports primary care providers to deliver safe, effective, and patient-centered care.
Key Responsibilities: Collaborate with patients, providers, and the care team to improve quality outcomes, perform clinical tasks within scope, and ensure accuracy and attention to detail.
Qualifications: Graduate of an accredited practical nursing program and licensed as an LPN in the practice state.
Skills and Abilities: Strong communication, teamwork, organizational, and interpersonal skills; customer service orientation; flexible, positive attitude; self-directed with minimal supervision.
Preferred Experience: At least 6 months clinical healthcare experience, preferably in primary care.
Physical Requirements: Ability to lift 50 pounds, stand and walk for extended periods, and perform tasks requiring reaching, bending, kneeling, twisting, and squatting.
